Memory Loss & Cognitive Decline: What Actually Helps

There's a real, well-studied list of daily habits tied to slower cognitive decline — and a real list of popular supplements the evidence specifically does not support. Both lists are worth knowing before changing anything.

Some forgetfulness is a normal part of getting older — misplacing keys, blanking on a name mid-conversation. What researchers actually track as concerning is a different pattern: memory or thinking changes that get worse over time and start interfering with daily tasks a person used to handle without trouble. The encouraging part of the real research here is that a meaningful share of that risk is tied to things people can actually act on.

See a Doctor First, Not Last

Sudden confusion, or memory loss that comes on and worsens over days to a few weeks rather than gradually over years, is not something to self-manage — it can point to something treatable: a urinary tract infection, a medication interaction, a thyroid problem, depression, a real B12 deficiency, or (less commonly) a condition like normal-pressure hydrocephalus, which can look like dementia but is often reversible when caught. The same goes for new confusion paired with a fall, a headache, or slurred speech. A gradual, years-long pattern is a different, less urgent situation — but still worth a real evaluation rather than guessing at the cause.

The real risk-factor list

The Lancet Commission on dementia prevention — a standing group of dementia researchers that periodically reviews the global evidence — currently lists 14 modifiable risk factors across a person's life, and estimates that addressing them could delay or prevent roughly 45% of dementia cases worldwide.[1] The factors most relevant day to day: uncontrolled high blood pressure, hearing loss, smoking, physical inactivity, diabetes, excess alcohol, social isolation, depression, obesity, untreated vision loss, and high LDL cholesterol. None of these are exotic — they're the same handful of things most general health advice already points to, which is part of why this list holds up as well as it does.

Hearing loss is a bigger factor than most people expect

Hearing loss shows up as one of the largest single modifiable factors on that list, and a large randomized trial (ACHIEVE) tested the idea directly: does correcting hearing loss actually slow cognitive decline? The result was genuinely nuanced, not a clean yes across the board — across the full study group, the hearing intervention didn't show a significant overall effect, but in the pre-specified subgroup of older adults already at higher cardiovascular risk, it slowed cognitive decline by roughly 48% over three years.[2] The honest takeaway: a hearing check is a low-risk, evidence-backed step, especially for anyone with existing heart-health risk factors — not a guaranteed fix for everyone.

What a real multidomain approach looks like

The most-cited controlled trial in this space, FINGER (Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability), didn't test one habit in isolation — it tested a combined program of diet changes, regular exercise, cognitive training, and management of vascular risk factors like blood pressure, in adults already at elevated risk. Over two years, the group following the combined program maintained cognitive performance measurably better than the control group.[3] The finding that matters most for a daily-rituals list: it was the combination that worked in that trial, not any single piece tested alone.

B vitamins: real evidence, in a specific population

A randomized, placebo-controlled trial out of Oxford (VITACOG) gave adults with mild cognitive impairment and elevated homocysteine a specific daily combination — 0.8mg folic acid, 0.5mg vitamin B12, and 20mg vitamin B6 — for two years, and found it measurably slowed the rate of brain atrophy compared to placebo.[4] That's a real, well-designed result. The caveat that matters: it was tested in people with mild cognitive impairment and elevated homocysteine specifically, at those specific doses — it's evidence for a targeted use, not a blanket "everyone should take high-dose B vitamins for memory" claim.

What the evidence specifically does not support: ginkgo biloba

Ginkgo biloba is one of the most popular "memory supplements" sold, and it's also one of the more rigorously disproven ones. The Ginkgo Evaluation of Memory (GEM) study — a large, long-running randomized controlled trial of over 3,000 older adults published in JAMA — found that ginkgo biloba did not reduce the rate of dementia or Alzheimer's disease compared to placebo, over more than six years of follow-up.[5] This is a case where the research question was asked directly, at scale, and came back negative — worth knowing before spending money on it for this specific purpose.

Everyday habits worth building into a routine

  • Move most days. Regular aerobic activity — brisk walking counts — is one of the most consistently replicated factors across this research, likely acting through both cardiovascular and direct brain-health pathways.
  • Protect sleep. Poor sleep, and untreated sleep apnea specifically, is linked to markers of brain change over time — this is one of the more actionable, least talked-about items on the list.
  • Stay socially connected. Regular social contact and engagement is its own named factor on the Lancet Commission list, independent of exercise or diet.
  • Keep learning something. Cognitive training showed real, if modest and skill-specific, long-term benefit in large trials — a crossword, a new skill, or a language app all fit this category; consistency mattered more than any one method.
  • Get blood pressure, blood sugar, and cholesterol checked and managed — all three are named risk factors, and all three are ordinary things a primary care visit already screens for.

Two memory exercises with real evidence, step by step

Beyond general "keep your brain active" advice, two specific techniques have real controlled research behind them — not just tradition.

The memory palace (method of loci)

This is a real, ancient mnemonic technique, and a 2017 study out of Radboud University put it through a controlled trial: participants who trained on it for 30 minutes a day over 40 days roughly doubled how many items they could recall, and brain scans showed their neural connectivity had shifted to resemble that of professional memory athletes — with the improvement still holding four months later, without further practice.[6] How to actually do it:

  1. Picture a place you know in detail — your home, your regular walk to the store.
  2. Mentally walk a fixed route through it, noticing 5–10 distinct spots along the way (the front door, the kitchen counter, a specific chair).
  3. To remember a list, "place" one item at each spot — the more vivid or unusual the mental image, the better it tends to stick.
  4. To recall the list later, mentally walk the same route in the same order and "look" at each spot.

Retrieval practice — quiz yourself instead of re-reading

Decades of controlled classroom and lab studies have found that actively trying to recall information — quizzing yourself — produces better long-term retention than re-reading the same material, a finding robust enough to have its own name in the research literature, "the testing effect."[7] In practice: after reading or learning something you want to keep, close it and try to recall the main points from memory before checking whether you got them right, and space those self-check sessions out over days rather than cramming them together.
